Membership Waiver and Assumption of Risk

In consideration of being allowed to participate in any way in the Brazilian Jiu-Jitsu instruction and other activities offered by Georgian Bay Brazilian Jiu-Jitsu (GBBJJ) including but not limited to Yoga, Pilates, Bootcamp Classes and weightlifting, hereinafter referred to as "Gym", I, the undersigned, do hereby agree as follows:

Assumption of Risk and Release of Liability

1. I understand that Brazilian Jiu-Jitsu, Yoga, Bootcamps, Pilates and weightlifting  are physical activities that involve a risk of serious injury, including permanent disability or death.
2. I acknowledge and assume all risks associated with participation in Gym activities, whether supervised or unsupervised, including but not limited to:
   • Injuries from falls, throws, takedowns, strikes, grappling techniques, and submissions.
   • Injuries arising from the actions or inactions of myself or others participating in Gym activities.
3. I voluntarily agree to participate in Gym activities at my own risk.
4. I release and hold harmless GBBJJ, its instructors, staff, agents, representatives, and other participants from any and all claims, demands, losses, or liabilities arising out of or in any way connected with my participation in Gym activities.
5. I understand that this waiver and release of liability extends to my heirs, assigns, and personal representatives.

Medical Release

1. I certify that I am in good physical and mental health and have no condition that would prevent me from safely participating in Gym activities.
2. I understand that I am responsible for consulting with a physician before participating in Gym activities if I have any concerns about my health or fitness.
3. In the event of an injury, I grant GBBJJ permission to obtain medical treatment for me if I am unable to do so myself.

Gym Rules and Regulations

1. I agree to abide by all Gym rules and regulations communicated by instructors.
2. I understand that I may be suspended or terminated from membership for violation of Gym rules or for conduct that is deemed unsafe or disruptive.

By signing below, I acknowledge that I have read and understood this entire waiver and agree to all of its terms.

Please Note:

• A parent or legal guardian must sign this waiver on behalf of anyone under the age of 18.
• A copy of this waiver will be provided to you for your records.
